Description
The subject of the project is the development and introduction of processes between the service sectors "health" and "social affairs", through which residents of nursing homes receive high-quality specialist medical care in their living environment, which can currently only be guaranteed by referral and transfer to the nearest hospital. The sensible use of digital tools ("telemedicine") in the target processes is expressly desired.
The aim of the project is to develop innovative specialist treatment processes on the basis of 2 (possibly 3) use cases, to implement them on a pilot basis and to evaluate them with regard to their benefits in relation to defined impact goals.
